| 説明 | The deposit of monosodium urate crystals in and around joints, and sometimes in the skin and soft tissues, is the basis of the clinical picture known as gout, which is associated with hyperuricemia. The presence of these crystals in the tissue produces a severe inflammatory reaction dominated by foreign body giant cells and histiocytes that form a type of foreign body granuloma surrounding the crystal aggregates.Tophi usually affect the olecranon bursa, as seen in the present case. Surgical specimen above left. On the right, in a section, it can be seen how the deposits are organized into small whitish or yellowish nests. In the remaining photos we see these nests at different magnifications, appreciating that some of them cystify, and also the macrophages that surround them or are inside them. In the higher magnification photos, the empty acicular spaces left by the crystals can be seen. Uric acid crystals require water-free processing because they are water-soluble and disappear with formalin fixation. In properly fixed tissue, the crystals are needle-shaped. In formalin-fixed tissue, the needle-shaped crystals are usually removed and only the empty space they occupied is visible, as in the photos of this case. Uric acid crystals are birefringent in polarized light, but in this case they were not because the crystals had disappeared.
