Figure 4-6 (page 4-11/4-12) from the Final Report of the Apollo 13 Review Board, dated June 15, 1970.NASA Caption: “Figures 4-5 through 4-8 are photographs of portions of the Apollo 13 service module (SM 109) at the North American Rockwell plant prior to shipment to KSC … Figure 4-6 shows the oxygen tank shelf, with oxygen tank no. 2 at left center." Notes: “Tank was originally installed in Apollo 10's SM, but was removed for modification and in process was dropped two inches (skin of tank is only 0.02 inch thick.) Then it was installed on Apollo 13 and certified, despite test anomalies.
